V-006 SIMULTANEOUS TREATMENT OF VENTRAL HERNIAS & GROIN HERNIA & RECTUS ABDOMINIS DIASTASIS BY LAPAROSCOPIC E-TEP REPAIR

Abstract Aims Ventral hernias are a frequent pathology in general surgery.Its association with rectus abdominis diastasis and groin is not uncommon.Currently, the minimally invasive approach of these pathologies becoming more frequent.We present usefulness e-TEP-approach to treat different types at same surgical-procedure. Methods Video-case presentation. Results 73-year-old-man,surgical-histories:umbilical-hernioplasty onlay-mesh lap-sigmoidectomy. Physical-examination:midline ventral-hernia(M2–3W2:8cm),diastasis(6cm) right hernia. CT-scan: midline ventral-hernia small-bowel into hernial-sac small defect(15mm) flank(L2).We decided perform laparoscopic-extraperitoneal-technique(eTEP). We use 4 ports(2×12mm, 2×5mm).The surgery begins dissecting left retromuscular space using dissector balloon.We continue supraumbilical cross-over transecting posterior-sheath then complete dissection other space.The continues down pubis, space.We right-TAR,at this area we observe lateral defect on preperitoneal-space.After that closure 2–0 V-lock anterior-sheath Monomax-loop-1.We placed 3D-PVDF-mesh for groin-hernia 30×40cm-PVDF-mesh fixed glue drainage.The patient had satisfactory evolution was discharged 2°POD. Conclusions The enhanced view totally-extraperitoneal (eTEP) an effective safe option cases ventral associate diastasis,as case presented.Posterior component separation-techniques(TAR) allow us achieve correct tension-free abdominal wall place wide mesh covering all hernial-defects.
